the Position: As a Youth Counselor, you will play a crucial role in
supporting the emotional, physical, and social well-being of
adolescent and youth residents. Working in small group settings,
you and your team will ensure that the individuals in our care
thrive and achieve their goals. The Counselor will provide targeted
support to help youth aged 10 to 19 who are facing disciplinary
issues at school, conflicts at home, or challenges within their
community. overcome obstacles and develop healthy coping
strategies. Primary Responsibilities/ Tasks: 1. Conduct
evidence-based focus groups utilizing the Positive Action
curriculum to help youth cope with issues related to family
dynamics, abusive histories, substance abuse, and other challenges.
2. Provide bi-weekly individual counseling sessions to each youth
in the caseload, addressing their specific emotional and behavioral
needs. 3. Offer additional support to youth and family members by
facilitating therapeutic sessions or referring them to appropriate
therapeutic resources. 4. Engage youth to evaluate the
effectiveness and impact of their involvement in the organization
and program, as well as its impact on their personal development.
5. Conduct pre- and post-assessments with each youth to measure the
impact of program outputs and outcomes. 6. Collaborate with a Case
Manager to conduct the initial intake process, either on-site or at
the youth’s home. 7. Develop Individual Service Plans (ISPs) and
regularly consult with the Case Manager and program staff regarding
each youth’s progress. 8. Participate in home visits and engage
with parents/guardians to discuss youth progress and necessary
actions. 9. Participate in monthly case review meetings with the
team and DHS Liaison, ensuring accurate tracking of each child’s
progress toward IPS-driven outcomes. 10. Collaborate with program
staff to achieve specific program goals, including: a. 85% of youth
completing at least 90 days of the program. b. 90% of participants
improving their social skills by the end of the program. c. 80% of
youth reducing violent behavior by the end of the program. d. 80%
of youth improving school attendance by the end of the program. e.
80% of youth who admitted to using drugs or alcohol reducing their
usage frequency. f. 100% of youth being connected to support
services addressing their referral reasons. g. 50% of youth being
connected to workforce development training. 11. Document all
services provided for each assigned case, ensuring accuracy and
confidentiality, including daily attendance. 12. Actively
participate in after-school activities, including assisting with
transportation, cooking, event coordination, outreach events, or
other duties as needed. 13. Participate in at least 20 hours of
training and professional development annually.14. Contribute to
Xiente community events and participate in quarterly strategic
meetings as required. 15. Complete any other duties as assigned by
the Student Success Coordinator. Organizational Competencies:
