Under the supervision of the PCMH manager, the PCMH analyst will evaluate and enhance opportunities for improvement through PCMH; educate and collaborate with providers, clinical staff, and non-clinical staff on the nuances of PCMH transformation; and facilitate implementation within practices whenever possible. The above activities should be grounded in a working familiarity with NCQA PCMH standards and guidelines. By coordinating NCQA knowledge with that of extant payers, the PCMH analyst will help reinforce practice compliance with PCMH. JOB DUTIES AND RESPONSIBILITIES: ESSENTIAL FUNCTIONS: 1. Supports continual instructional efforts and educates staff to bolster compliance relative to standards and guidelines for PCMH. 2. Initiates and aids in report generation as required. 3. Assesses practice PCMH readiness, monitors regularly, and accurately reports evaluation results for each participating practice. 4. Stays current with ongoing improvement initiatives pertaining to PCMH and the broader healthcare environment. This will also entail evolving knowledge of PCMH reimbursement potential for each relevant payer. 5. Appropriately researches strategies for improvement within practices and reviews findings with PCMH manager. 6. Maintains high level of competency in physician group electronic health record and practice management system, and works closely with Information Technology staff for purposes of electronic reporting of PCMH and quality data. Aids in the generation and implementation of short and long-term improvements and objectives for PCMH success. 9. Communicates openly and frequently with interdepartmental staff to foster team-based solutions pertaining to PCMH. 10. Facilitates PCP practice transformation to the PCMH Care Model by focusing on optimizing PCMH value, including clinical and financial success. 11. Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information. 12. Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. 13. Ensures patient satisfaction in all areas of the practice. 14. Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. 15. Complies with Network and departmental policies regarding attendance and dress code. OTHER FUNCTIONS: 1. Other related duties as assigned. PHYSICIAL AND SENSORY REQUIREMENTS: Requires sitting, standing and walking for extended periods of time (up to 8 hours at a time). Requires continual use of fingers for writing and computer entry. Routine use of upper extremities; occasional requirement to lift up to 25 pounds. Regularly entails abilities to stoop, bend, and reach above shoulder level. Requires ability to hear normal conversation and good general, near, and peripheral vision. POTENTIAL ON-THE-JOB RISKS: None identified SPECIFIC PROTECTIVE EQUIPMENT AVAILABLE: N/A MOST COMPLEX DUTY: Assist the PCMH manager in PCMH transformation within PCP practices, utilizing NCQA standards and guidelines as a framework for implementation. This will necessitate meeting or exceeding the requirements set by NCQA, as well as the requirements established by involved payers. The position requires the ability to prioritize and to frequently work under time and resource constraints with a diverse population. SUPERVISION (RECEIVED BY AND/OR GIVEN TO): Supervised by the PCMH Manager COMMUNICATIONS: The PCMH Analyst engages in daily written and verbal communication with any and/or all of the following: SLUHN personnel, pharmaceutical representatives (educational division), and/or third party insurance carriers. Excellent verbal and written skills are required. ADDITIONAL REQUIREMENTS: Demonstrates mature, conscientious, and professional behavior. Performs duties with minimal error and within appropriate timeframes. Capably interprets, adapts, and applies guidelines and procedures. Accepts responsibility and follows directions. Maintains positive interpersonal and interdepartmental relationships. Uses good reasoning and judgment and reacts calmly in emergency situations. Possesses the abilities to read, write, and communicate effectively verbally and in writing. Proficiency in the operation of a computer keyboard. MINIMUM QUALIFICATIONS EDUCATION: Associates Degree or three years experience as clinical staff in a medical office or clinical setting TRAINING AND EXPERIENCE: Knowledge of PCMH standards and guidelines under NCQA. Knowledge of general medical terminology. Experience with electronic medical records. Expertise with MS Office (Word, Excel, Power point, and Outlook) preferred.
