Individually and together, our employees are dedicated to satisfying the mission of our organization which is an unwavering commitment to excellence as we care for the sick and injured; educate physicians, nurses and other health care providers; and improve access to care in the communities we serve, regardless of a patient's ability to pay for health care.The Family Development Specialist will provide student and family case management focusing on reducing chronic absenteeism and measurable connections to care for priority needs identified through St. Luke's Community Health Needs Assessments and intake assessments within the Panther Valley School District.

  • Provide case management and data collection and analysis
  • Provide one-on-one follow-up with students and outreach to families
  • Collect and review critical data pertaining to students (academic, attendance and behavior), families and programs. Analyze and track data to develop new strategies and plans for continuously improving existing interventions
  • Attend/facilitate attendance team meetings
  • Assist with in-school competitions and programs and assist in the creation of individual attendance plans
  • Work to support the Community School Coordinator/Community Health Liaison Manager and Community Health Team with whole school attendance awareness campaign
  • Adhere to the guidelines and outcomes established to meet the terms of the grant
  • Generate quarterly reports on the outcomes of the Community School model to various stakeholders, including, but not limited to, the United Way of the Greater Lehigh Valley, St. Luke's Hospital, Panther Valley School District and other potential sponsors of the Community School program
  • Initiate home visits on a need be basis
  • Conduct an interview/intake process to determine the needs of enrolled students with their families in this program. Make concerned efforts to measurably connect and track the student & family to resources
    Sitting for up to eight hours per day, up to two hours at a time. Walking for up to one hour per day up to 15 minutes at a time. Involves frequent fingering, firm grasping, handling and twisting and turning. Occasional lifting and carrying objects up to 25 pounds. Occasional stooping, bending and reaching above shoulder level. Hearing as it relates to normal conversation. Seeing as it relates to general vision, color vision, peripheral vision and visual monotony.
    Four year degree in social work required, Master's Degree in related field preferred.
    Five years of experience in case management service or other experience as related to site of service required. Training in cultural competency and trauma informed schools, Community Health Worker (CHW) preferred. Bilingual (English/Spanish) preferred.
