You will work alongside a supportive Director of Nursing and Administrator who value partnership and leadership. The facility is proud of its staff longevity and strong culture of teamwork and trust. Your Role: Lead and oversee the MDS program, ensuring accuracy and compliance. Optimize PDPM processes and reimbursement opportunities. Provide leadership, mentoring, and ongoing support to MDS Coordinators. Collaborate with nursing and interdisciplinary teams for comprehensive care planning. Promote a culture of continuous learning, accountability, and innovation. What We Are Looking For: RN license preferred. Proven experience in long-term care and MDS coordination. In-depth knowledge of MDS 3.0, PDPM, Medicaid/Medicare, and EMR systems. Strong leadership and communication skills. A collaborative, team-first mindset and dedication to resident care.
