Title: Community Follow-up Worker Department: Health Home – DSRIP Reports To: Case Manager Location: Bronx, NY Position Status: Part-Time FLSA Status: Non-Exempt POSITION SUMMARY Community Follow-up Workers are members of Brightpoint Health programs who participates in a special service delivery opportunity. ESSENTIAL FUNCTIONS General Peer Educator Tasks: Carries out tasks that are needed to execute the medical and support service plans Accompanies patients to appointments when required, provides coaching to patients, delivers monthly or weekly health education encounters Maintain client-related records and other required documentation according to the protocols and standards Providing education Providing one-to-one peer support Assisting with support and socialization groups Assisting in data collection and data entry Completing program paperwork Making referrals to internal and external programs jointly with case manager Other tasks as assigned Commitment Maximum 20 hours per week. It is not to be construed as an exhaustive statement of all the job functions EDUCATION/EXPERIENCE: Requirements Conduct assigned weekend and evening outreach Travel to different areas of the Bronx for service activities Be open to learning Must be able to attend mandatory trainings, and individual and/or group supervision as directed by their peer supervisor Be a team player LANGUAGE SKILLS: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
