Job Description:
Job Description Post Acute Care Physician opportunity in Media,
PA Great full-time (5 days per week) primary care role, you will
have the opportunity to manage complex patient cases in post-acute
settings (skilled nursing and independent living facilities) in
Media, Pennsylvania, focusing on improving care for some of the
most vulnerable patients. Position Highlights: Uncapped earning

facilities- Flexible rounding schedule Manage chronic disease,
diagnose illnesses and medical conditions, and implement plan of
care Provide oversight, guidance, and mentorship to our nurse
practitioners Conduct clinical case reviews, as needed and share
tools, information, and guidelines as they relate to quality of
care Qualifications: Minimum of 1 year of experience, with a
commitment to delivering high-quality, patient-centered care
Post-acute care experience is beneficial, though not required, as
we provide comprehensive support and development opportunities
Active and unrestricted Pennsylvania license (or in process) Active
and unrestricted DEA/prescriptive authority (or in process)
Internal medicine (IM), family medicine (FM), emergency medicine
(EM), hospitalist or candidates with any related experience are
welcome to apply Compensation: Compensation is fee for service
(FFS). Expected compensation is an estimated range of $180,000 to
$291,000 annually with no cap on productivity income potential. Job
