Job Description:
ChristianaCare is seeking a dedicated and compassionate Licensed
Practical Nurse (LPN) to join our Home Health team on a per diem
basis, providing up to 80 hours of service per month across
Delaware. This role is ideal for a nurse seeking flexibility and
the opportunity to make a significant impact in the lives of
patients in their homes. The LPN will work under the supervision of
a Registered Nurse or doctor, performing duties such as monitoring
vital signs, administering medication, and assisting with daily
living activities to enhance patient health and recovery. Monitor
and record patients' vitals and prepare and administer medications
as directed by the healthcare provider. Assist with basic patient
care activities, including bathing, feeding, and dressing. Observe
and report any changes in the patient's condition to the
supervising nurse or physician. Educate patients and their families
on proper home health care strategies and procedures. Coordinate
with healthcare providers to ensure continuity of care. Manage and
maintain accurate patient records and documentation. Strong
clinical skills and knowledge of healthcare protocols Excellent
communication and interpersonal skills Proficiency in using medical
software and record-keeping Ability to work independently and
manage time effectively Medical Insurance Dental Insurance Vision
Insurance Life Insurance Current LPN license valid in the state of
Delaware CPR certification required
