You will play a crucial role in ensuring a seamless transition to managed long-term care.Key Responsibilities

• Conduct outreach activities within the community to assess the eligibility and safety of potential members.
• Collaborate with the Intake/Outreach team and clinical team to ensure appropriate services are provided to new members.
• Communicate effectively with potential members, families, referral sources, and physicians.Qualifications
  • Current New York State Registered Nurse License.
  • Bachelor's degree in Nursing preferred.
  • One (1) year of experience in an acute care setting.
  • Must have UAS-NY experience.
  • Home care and management experience preferred.
  • Proficient in MS Office.
  • Strong oral, written, and interpersonal communication skills.
  • Bilingual in Chinese or Spanish required.Compensation
    • Annual salary: $95,000 - $105,000
    • Eligible for a 20,000 sign-on bonus ($5,000 payable every six months, less taxes)
