Job DescriptionEklego is seeking a RN Case Manager for our client's Home Health division. This is a direct hire opportunity with a nationally recognized organization!What will I do as an RN Case Manager? This is a direct-hire, full-time position, working Day time hours Monday-Friday, Occasional Weekends and monthly on-call rotation. Weekend Programs available. The Registered Nurse Case Manager works independently throughout Allentown, PA, providing initial and ongoing assessment of client's skilled nursing needs. (Counties include: Lehigh, North Hampton, Monroe and Allentown) The RN Case Manager will also implement care plans and make referrals to other services that support the patient's care plan. You'll also be provided with all the supplies you'll need for documentation and treatment.

Requires:

• Current RN license for Pennsylvania
• Valid Driver's License and reliable vehicle
