The primary responsibilities of this position is assisting the oncology patient by connecting them with resources and support services, coordinating and scheduling appointments, assisting with medical paperwork, obtaining medical records and reducing fear and anxiety. The Oncology Care Coordinator will assist the multidisciplinary team with providing individualized care to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care from diagnosis through all phases of the cancer experience. JOB DUTIES AND RESPONSIBILITIES:

  • Provides assistance to patients and families by guiding them through navigation process. Schedules patient appointments, explains steps and procedures, gathers imaging films and pathology slides from outside facilities. Communicates extensively with all members of team on behalf of the patient
    • Answers incoming telephone calls to determine the purpose of a call, and to forward the call to appropriate personnel or department. Well versed and able to answer a variety of questions about services and programs. - Complete new patient intake forms
      • Provides patient assistance with a vast variety of services including transportation, support group information, referrals to other oncology team members (oncology nurse navigators, financial counselors, social workers) and referrals to appropriate community resources
        • Maintaining metrics and reports (tracking and running reports on specific values for specific disease sites), sending out new, disease specific patient education binder/materials to patients, sending out patient satisfaction surveys and tracking the results
          • Maintain comprehensive, timely, and legible records capturing ongoing patient barriers, patient interactions, barrier resolution, and other evaluation metrics and report data
            • Build trust by being accessible, accurate, supportive, and acting within scope of practice. - Apply knowledge of the difference in roles between clinically licensed and non-licensed professionals and act within professional boundaries
            • Demonstrate a commitment to ethical principles pertaining to confidentiality, informed consent, business practices, and compliance with relevant laws, policies, and regulations (e.g., HIPAA, agency abuse reporting rules)
            • Assist patients in accessing cancer care and navigating healthcare systems. Assess barriers to care and engage patients and families in creating potential solutions to financial, practical, and social challenges
            • Attending disease specific multidisciplinary case reviews (tumor boards) in order to stay engaged and up to date with new developments on patients and to better assist the team/nurse navigator with patient care PHYSICAL AND SENSORY REQUIREMENTS: Sit up to 7 hours per day; 3 hours at a time. Stand for up to 3 hours per day; 2 hours at a time. Walk 1 hour per day; 10 minutes at a time. Frequently lifting, carrying and pushing objects up to 10 pounds. - Rarely lifting, carrying and pushing objects up to 75 pounds. Frequently stooping and bending. - Frequently reaching above shoulder level. Must be able to perceive attributes of an object through touch. - Must be able to hear as it relates to normal conversation, high and low frequencies. Must be able to see as it relates to general near, far, color and peripheral vision. - Must be able to speak as it relates to normal conversation and projection as it relates to lectures and presentations. Long periods of computer use.
