Job Description:
Valley Youth House empowers and strengthens the lives of
children, youth and families through inclusive programming that
builds resilience and fosters growth and independence.We are
currently looking for a qualified applicant to fill the following
position: FT - Life Skills Counselor - Case Manager ($18hr -
$21hr).Job Description: The Life Skills Case Manager is responsible
for client supervision and guidance, case management, individual
and/or group life skills learning, and all aspects of day-to-day
program functions, which may include apartment and site monitoring,
community relations issues, and liaison-related contacts. This role
functions as a parent would. This also includes generating goal
plans with youth to track their accomplishments, submitting all
required county, state and federal documentation. As well as
maintaining all documents related to the client records.Essential
Functions:Successfully engage youth in developing and implementing
their individual goal and/or transition plan using positive youth
development, trauma-informed and culturally appropriate
frameworks.A minimum of 50% of the working hours will be spent
providing quality in-person goal planning, instruction and case
management services as a parent would according to the youth's
identified level of service.Ensure up-to-date paper & electronic
client records in compliance with program, county, state and
federal regulations and private grant requirements as
applicable..Key Functions:Meet with the youth according to program
requirements to model and practice independent living skills with
youth, including, but not limited to: academic and career planning,
permanent relationships/mentoring, housing/household maintenance,
priority setting, money management, cooking and nutrition, and
health and wellness.Represent the program and agency in a
professional manner and maintain positive community relationships
in a proactive manner.Be the primary liaison contact and provide
regular ongoing case coordination with community contacts such as:
referral/funding sources, children and youth agencies, landlords,
probation, school personnel, employers, and doctor's
offices.Participate in on-call rotation as required by the program
area.Address and report any crisis or critical incident as defined
by agency and/or county, state or federal regulations according to
the agency's formalized practice and protocols.Other
Duties:Understand and adhere to Valley Youth House policies,
practices and procedures.To participate in weekly scheduled
supervision with the direct supervisor and be prepared with an
agenda.Attend and actively participate in team meetings.Attend both
external and internal agency training and workshops and complete
and pass all required training as set forth in the funder and
licensing regulations as required.To complete other related tasks
as appropriate or assigned.Minimum Qualifications:An individual 21
years of age or older, with the ability to identify with and adapt
to the goals and philosophies of the independent living program.
Candidates with lived experience may be considered.Possess sound
judgment and knowledge of adolescent development and
behavior.Display organizational skills and highly adept at managing
multiple tasks and assignments.Successful team members are creative
thinkers, are flexible in their approaches to people and projects,
and are capable of working with diverse groups of the
population.Must have and maintain a valid driver's license
including vehicle insurance and inspection on any vehicle used for
daily work and transportation of clients.Must receive a passing
score on annual motor vehicle clearance to be eligible to operate
agency vehicles.The ability to navigate stairs is highly preferred
(as some locations are not always handicap accessible).Proficient
in computer knowledge & skills (i.e. Microsoft Windows software);
familiarity with the operations of external databases and Google
Suite is desired.Never have been convicted of any prohibited
criminal or child abuse offense.They will possess a minimum of a
BS/BA in social work or a similar degree in human services as well
as three years of related experience in the field.Preferred
Qualifications:Bachelor's Degree in Human Services related field.At
least one year of working with teenagers.Group facilitation
experience.Physical Requirements:Sedentary work that primarily
