Now through December 2022, CFF is offering a $1,500 sign on bonus*; $500 paid on first check, $500 on the paycheck following 6 months, and $500 on the paycheck following 1 year of successful employment.*Bonus amounts could be prorated based on number of hours workedPosition Summary:First Episode Psychosis is a Coordinated Specialty Care (CSC) approach to treating young people who have recently experienced their first episode of psychosis (FEP). The FEP Team offers young people an array of services, including low-dose medication management, Cognitive Behavioral Treatment for Mental Health and Psychosis, Family Education, Case Management, Supported Education and Employment, and peer support services. Service coordination is guided by the young person's voice and choice. FEP seeks to improve the quality of life of young people by instilling hope through empowerment to guide their own treatment, educating them about their psychosis, re-establishing relationships, and re-integrating them back into the community whether it's attending school or working.This position is responsible for developing, coordinating, and implementing services for consumers as identified in the service plan. Responsibilities include providing case management, service planning, and coordination of services with other agency providers to meet the client's needs along with supportive counseling. The FEP Case Manager will also provide advocacy to consumers in order to help consumers set and achieve recovery goals and empower consumers to increase self-advocacy.This candidate will work on a closely coordinated multidisciplinary team to provide community-based support to a caseload of up to 30 adolescents and young adults between 15-30 years old who are experiencing their first episode of psychosis.Program Specific Responsibilities:Assist in the completion of comprehensive assessments and treatment plans for each individual client.Complete a written strength assessment, develop and implement a written service plan for each consumer assigned to the caseload.Provide services that diffuse or resolve crises in order to prevent unnecessary use of emergency rooms, in-patient services and mental health court.Make home, school, hospital and community based visits at a minimum of every 30 days (or more often as determined by the consumer's level of care).Familiar with and able to access community resources and child serving systems in Delaware County.Achieve billable hour requirements per week.Assume case load of up to 30 clients.Transport clients and families to various appointments based on caseload as needed.Document supervision/case activity through ETO, progress notes and other program specific paperwork by typing or handwriting.Ensure that program files are organized and paperwork is filed into client specific charts.Provide on call rotation for 24/7 mental health crisis support.May perform other related duties, when required or assigned.Required Qualifications:Bachelor's degree in Human Service or related fieldMinimum of one year direct clinical experience in mental health services working with children, youth and familiesCurrent child abuse, criminal and FBI clearances.Maintain valid driver's license and car and the ability to travel in your own vehicle to off site locations as required by caseloadPreferred Qualifications:Experience working with Transition Aged YouthKnowledge of resources in Lehigh County and surrounding countiesBi-lingual in Spanish LanguageCrisis management experienceExperience working with Schizophrenia SpectrumCompensation, Benefits and Schedule:Hourly rate of $17-18 an hourVaried work schedule, evening and weekends may be required based on client needs.Medical and Vision Benefits offered on first day of employment, 3 plan options offered by Independence Blue CrossDental Benefits offered on first day of employment, 2 plan options offered by GuardianAncillary Benefits offered on first day of employment, such as Accident, Critical Illness, Cancer, Short Term DisabilityLong Term Disability and $50,000 Life Insurance Policy provided by CFF9 Paid Holidays17 days of Paid Time Off with additional days added through years of servicePaid Parental LeaveTuition ReimbursementAnnual Salary Increase and Bonus401K plan with company match and profit sharingMileage and Expense reimbursement for travel and trainingClearance Reimbursement*Eligibility Requirements applyWe provide equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, sex, national origin, age, disability, veteran status, gender identity, or genetics.
