Job Description Salary Range: $35,560-$38,000/Annually Patient
Care Coordination (PCC) is a fast-growing Service Coordination
agency located in central Philadelphia, PA. We provide supports
coordination in the Philadelphia and surrounding counties for
Medicaid eligible participants who are chronically disabled and/or
of an aging population. PCC---s goal is to help our participants
maintain their independence in the community through connecting,
coordinating, and monitoring services and resources that can
ultimately prevent them from being hospitalized or in a nursing
home while staying in their own homes for greater quality of life.

qualified, passionate Service Coordinator for a full time position
that will work from home several days per week while spending on
average one or two days per week in our Philadelphia office.
Training will be required and completed in the Philadelphia office.
The SC will manage a caseload of participants that is spread
amongst the following counties (Berks, Carbon, Lancaster, Lehigh,
Northhampton, Schuykill, York, Susquehanna, and Northumberland
counties) to help them achieve the ultimate goal of independence
through high-quality care and efficient delivery of community
resources. The ideal candidate will live in or near Reading or
Allentown to be central to these county settings. Job
Responsibilities Include: - Assessment and Service Plan
Development, Service arrangement, follow up and monitoring and
reassessment. Validates the assessment of the participants needs in
their environment. Reassesses participants status and reviews
service plan at regularly scheduled intervals. - Completes Care
Management Instrument at required timeframes - Works with
participant, family and caregiver to develop an Individual Service
Plan, making use of supervision and consultation with other
disciplines as necessary - Identifies and mobilizes informal and
formal resources to meet participant needs. Maximizes use of third
party payers - Facilitates participants' choice of providers -
Arranges for need services and entitlements, working cooperatively
with participant, family members, and service providers. Follows up
on service delivered in specified amount of time, and works with
consumer and provider to assure appropriate match of service to
specific need - Conducts home visits and telephone contacts per
standards - Completes all necessary forms and / or data entry for
participants case record, other management information, and other
written reports as required. Inputs necessary data and maintains
automated programs. - Participates in orientation and training,
in-service training as assigned, and attends regularly scheduled
supervision, and staff meetings - Monitors participant satisfaction
to ensure quality of services provided - Other duties as needed
Requirements: - Bachelor's Degree preferred in Human
Services/Social Services field. Other degrees may be considered
pending review and approval - Knowledge of state computer software
system (HCSIS / SAMS) is ideal - Knowledge of MCO computer system
(ELTSS / Envolve) preferred - Minimum 2 years experience in human
services/case management preferred - Proficient in MS Office
Applications (especially MS Office and Excel) - Local travel
Required (up to 50%) - Bilingual candidates in Spanish are a plus.
