Coordinator (RNAC)* to maintain the standards of Nursing Care and
practices within the facility to meet the physical, psychosocial,

patients every day. *Job Summary* The primary purpose of this
position is to provide management of resident care as related to
the Resident Assessment Instrument (RAI) process, in accordance
with federal and state regulations, and as directed by the
Administrator. The CCC is expected to have access to and follow the
most recent version of the RAI User's Manual at all times. The CCC
is accountable for helping achieve the organization's goals for
positive outcomes. This is to be achieved through effective
processes, monitoring, training and implementing systems that will
lead to appropriate MDS completion and submission, care planning,
billing practices. Utilization of resident's benefits and accurate
reporting of quality measure data. *Essential Job Specific Duties
and Responsibilities* 1. Conduct initially and periodically a
comprehensive, accurate standardized, reproducible assessment of
each resident upon admission, annually (not less frequently than
every 366 days) and whenever a significant change is identified in
the resident's status as defined in chapter 2 of the RAI User's
Manual. 2. Assess every resident using the quarterly item set not
less frequently than once every 92 days. 3. Conduct PDPM
assessments, including timely opening and completion, as outlined
in Chapters 2 and 6 of the RAI User's Manual. 4. Ensure clinically
accurate coding and completion of the MDS assessments utilizing MDS
verification software. 5. Provide oversight of assessment
completion and transmission to the national repository. 6. Review
final validation reports and corrections or modifications in
response to warnings or errors as needed. 7. The baseline care plan
will be reviewed prior to issuance to resident and/or resident
representative according to Regulatory regulation. 8. Assign and
coordinate the work of health professionals toward accurate and
timely completions of MDS assessments and care plans. 9.
Communicate all MDS schedules and schedule changes to the IDT. 10.
Be highly involved in determining skilled level of care for
Medicare residents and procuring required Medicare-specific
documentation; be responsible for physician certification of a
skilled level of care through the Medicare stay; be involved in
making eligibility determinations utilizing Dorado forms and
clinical documentation. 11. Coordinate care with case managers for
residents utilizing managed care, health maintenance organizations
(HMOs), commercial insurance, and other alternate payment models.
12. Conduct weekly utilization review meeting to ensure holistic
care of residents and assist in developing a safe discharge plan.
13. Ensure compliance with state-specific regulations regarding the
RAI process. 14. Obtain Quality Measure, SNF QRP and Five Star
reports at least monthly to review with the clinical team. Provide
insight and analysis of MDS-based Quality Measures. 15. Lead
Quality Rounds weekly to identify Residents with potential clinical
changes. 16. Serve as a member of the quality assessment and
assurance (QAA) and/or the Quality Assurance and Performance
Improvement (QAPI) committees. 17. Work closely with Facility
Admission Directors and Physicians to obtain accurate and complete
documentation to support ICD-10 diagnosis coding and surgical
procedures. 18. Participate in Pre-Admission Huddle, enter ICD-10
codes in PCC, identify Primary, Secondary, Third, etc. 19. Audit
and improve staff education/competency as needed to ensure accurate
and timely completion of supporting documentation and MDS
assessments. 20. Participate in 48-72 hour meetings, care
conference to ensure revision of care plans, discharge planning to
determine discharge goals, and the provision of resources as
needed. 21. Participate in facility Triple Check to ensure accurate
and timely billing. 22. Foster effective working relationships and
build consensus. 23. Maintain confidentiality of sensitive
information. 24. Plan, organize, prioritize, work independently,
and meet deadlines. 25. Use judgment and make sound independent
decisions. 26. Work effectively with individuals at all levels of
the organization, as well as with residents, family members.
Visitors, government personnel, and the public. 27. Speak, read,
write, and understand English effectively at a level appropriate
for the safe and effective performance of the job. 28. Be
knowledgeable of regulations, practice standards, and procedures,
as well as laws, regulations, and guidelines pertaining to the RAI
process. 29. Possess the ability to plan, organize, develop,
implement, and interpret the programs, goals, objectives, policies
and procedures, etc., that are necessary to ensuring the accurate a
timely completion of the RAI documents. 30. Have the ability to
learn computer systems, system applications and related office
equipment. 31. Ensure that the resident' rights to fair and equable
treatment, self-determination, individuality, privacy, property,
and civil rights, including the right to wage complaints, are
always well-established and maintained. *Job Requirements * 1. Must
be a Registered Nurse (RN). Licensed Practical Nurse (LPN) may be
hired in secondary 2. Clinical Care Coordinator Position with RN
oversight. 3. MDS experience or certification is preferred. 4.
Desired traits: integrity, attention to detail, accuracy,

Working Conditions* - Work in well-lighted/ventilated office areas,
as well as throughout the nursing service areas. - Sitting,
